Who we are

Transform Personal Training is a personal training studio at 2900 N Broadway St, Chicago, IL 60657. This policy explains what information we collect through this website, how we use it, and the choices you have. Questions are always welcome at contact@transformpersonaltraining.com.

Information we collect

When you fill out a form on this site, we collect what you type in: typically your name, email address, phone number, and anything you tell us about your goals. If you apply to join our team, we also receive the resume and details you submit. When we schedule sessions or exchange messages with you, we keep those communications so we can serve you well. This site does not ask you to create an account or password.

How we use your information

We use your information to respond to your inquiry, schedule your free consultation and sessions, deliver coaching and nutrition guidance, and send you related updates. With your consent, we may also send occasional offers or fitness content. We use general, non-identifying information about how the site is used to improve it.

Text messages and email

If you provide your phone number and check the consent box on a form, we may text you to schedule and support your training. Message frequency varies and message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. Our emails include an unsubscribe link, and you can opt out whenever you like without affecting your training.

Services we rely on

Our forms, scheduling and messaging run on HighLevel (LeadConnector), which processes form submissions on our behalf. The embedded form widgets include spam protection and may set their own cookies or use advertising and analytics pixels inside the widget. Existing members who book through the Members link use Mindbody, which has its own privacy policy. Our website hosting provider processes standard server logs, such as IP addresses, needed to serve the site securely.

Cookies

The site itself is a simple static website. Embedded third-party widgets, like our forms, may use cookies as described above. You can control cookies through your browser settings at any time.

What we don't do

We do not sell your personal information, and we do not share it with third parties for their own marketing.

Keeping and deleting information

We keep your information for as long as we have a coaching relationship with you, or as needed to respond to your inquiry and meet legal obligations. If you'd like us to access, update, or delete the information we have about you, email contact@transformpersonaltraining.com and we'll take care of it.

Children's privacy

This website is not directed to children under 13, and we do not knowingly collect personal information from them.

Changes to this policy

If we change this policy, we'll post the updated version on this page with a new effective date.
